In our efforts to provide comprehensive physical therapy services, we ask that you become acquainted with our office policies. If at any time you have questions, please feel free to ask. All recommended treatments are in the best interest of our patients. We do not allow insurance companies to dictate treatment. You will be informed of our fees BEFORE therapy begins. Finally, to help you receive the treatment you need, we can assist you with payment options.

  • Financial Options
    Peak Performance accepts personal checks, cash, Visa, MasterCard, private insurance, Medicare, Medicaid (patients under 21) and verified worker's compensation insurance as remuneration for treatment rendered. Applicable deductible amounts and/or co-payments, whether fixed amount or a percentage of charges, are due at each visit. Individual payment plans are also available.

    Peak Performance now accepts ONLINE payments securely using The Pay Plan system. Secure payments can be made from credit cards (Visa, MasterCard) and also checking accounts.

    Peak Performance is continuously seeking ways to improve the experience of our patients in all areas, to include dealing with the complexity of insurance benefits and payments. To that end, Peak Performance has made a significant investment in a new in-house billing system as well as hired an experienced billing staff.

    Peak Performance patients are now able to quickly obtain help on insurance and billing information by calling our local, friendly and helpful billing staff. Billing statements will list a new toll free number to make it easy for patient to contact the Peak Performance Billing staff. Patients can also speak directly with our Billing staff, collocated with the New Bern Peak Performance clinic.

    The most important improvement patients will experience is the ability the Billing staff’s ability to resolve issues quickly and directly.

    At Peak Performance our primary goal is the wellness of our patients. To that end, we work closely with our patients on their specific financial and insurance needs in order that our patients can concentrate on improving their health. Our dedicated, experienced professionals will coordinate your insurance coverage, explain benefits and help resolve any related problems. Contact Peak Performance and discuss your insurance coverage prior, during or after your therapy.

    First visit patients should plan on arriving 15 minutes early in order to complete medical history and insurance forms. To save time, you can also download and complete the required forms from the link below. If you chose to download and complete the form before arriving for your first appointment, please fully and accurately complete each form to include signatures. Please call or email us if you have any questions. Our Insurance specialist will confirm your insurance coverage and explain your benefits at the time of your first visit. We will file your insurance claims on a daily basis and bill you for non-covered cost.
  • Insurance
    Peak Performance accepts assignment of estimated insurance benefits as a courtesy to our patients, provided you submit a completed original insurance claim. Please note that your insurance is a contract between you and the insurance company--not between your Physical Therapists and the insurance company. Our usual and customary fees are a reflection of our commitment to excellence, which is in line with this geographical area.

    If for any reason your insurance company fails to pay for services rendered, you are responsible. Co-payments and deductibles are due when service is rendered. We are happy to answer any questions that you may have regarding specific insurance procedures.

    We understand that health information about you and your health care is personal. We are committed to protecting your health information. We create a record of the care and services you receive from us. We need this record to provide you with quality care and to comply with certain legal requirements. We also share your health information with other health care providers and insurance companies in order to meet your health care needs and financial obligations.

    In compliance with new government health industry privacy requirement (HIPAA), Peak Performance provides each patient a complete document of specific privacy requirements. We require each patient review and acknowledge our security and privacy policy.
