accepts personal checks, cash, Visa, MasterCard, private insurance,
Medicare, Medicaid (patients under 21) and verified worker's
compensation insurance as remuneration for treatment rendered.
Applicable deductible amounts and/or co-payments, whether
fixed amount or a percentage of charges, are due at each visit.
We also can develop a special payment plan.
Peak Performance now accepts ONLINE payments securely using
The Pay Plan system. Secure payments can be made from credit
cards (Visa, MasterCard) and also checking accounts.

At Peak Performance our primary goal is the wellness of our patients. To that end, we work closely with our patients on their specific financial and insurance needs in order that our patients can concentrate on improving their health. We have a dedicated, experienced professional who will coordinate your insurance coverage, explain benefits and help resolve any related problems. Most insurance carriers require a doctor's referral and a written prescription as well as preauthorization/ precertification. Contact Peak Performance and discuss your insurance coverage prior, during or after your therapy.

First visit patients should plan
on arriving 15 minutes early in order to complete medical history
and insurance forms. To save time, you can also download and complete
the required MS Word forms from the link below. If you chose to
download and complete the form before arriving for your first appointment,
please fully and accurately complete each form to include signatures.
Please call or email us if you have any questions. Our Insurance
specialist will confirm your insurance coverage and explain your
benefits at the time of your first visit. We will file your insurance
claims on a daily basis and bill you for non-covered cost.
